HIPAA Notice of Privacy Practices
Bridge Point QRTP respects the privacy of all individuals we serve. Our Notice of Privacy Practices describes how we may use and disclose protected health information (PHI) and how individuals can exercise their rights under HIPAA. Non-Discrimination & Civil Rights Notice
Bridge Point QRTP complies with applicable Federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, religion, sexual orientation, gender identity, or sex. We provide free aids and services to people with disabilities and free language assistance to people whose primary language is not English.

Client Rights & Grievance Notice
Individuals receiving services at Bridge Point QRTP have rights protected under Ohio Administrative Code 5122-30-22. Our Client Rights and Grievance Policy outlines these rights and explains how to file a complaint or grievance.

Licensure Disclosure
Bridge Point QRTP is licensed by the Ohio Department of Mental Health and Addiction Services (Ohio MHAS) as a Qualified Residential Treatment Program serving youth ages 12–18.

CARF Accreditation Statement
Bridge Point QRTP is accredited by CARF International for Residential Treatment Programs. This accreditation demonstrates our commitment to meeting internationally recognized standards of quality and continuous improvement.

Family Engagement & Permanency Statement
Bridge Point QRTP supports family involvement in all aspects of treatment and transition planning. We encourage regular family visitation, participation in therapy, and coordination with child welfare teams to promote permanency outcomes.
